Postpartum and NICU Care for Multiples: Understanding the Landscape

The postpartum period after a multiples birth is categorically different from a singleton postpartum. Higher rates of cesarean delivery, greater maternal physiological demands, an elevated probability of NICU admission for one or more babies, and the emotional complexity of caring for multiple newborns — possibly in different physical locations — create a unique and often underrepresented postpartum experience. Expecting parents of multiples benefit enormously from understanding this landscape before they are in the middle of it. By the end of this course you will be able to describe the physiological aspects of postpartum recovery that are specifically affected by multiples pregnancies and births, explain the basic structure and function of the NICU, describe kangaroo care and its role in premature infant development, and identify the key challenges parents face when navigating concurrent maternal recovery and NICU involvement. What you will learn: - The physiological dimensions of postpartum recovery after a multiples birth: uterine involution with greater distension, the higher risk of postpartum hemorrhage, and cesarean recovery considerations unique to carrying and delivering multiples - The NICU environment: the different levels of NICU care, the clinical terminology parents encounter, and how care team communication is typically structured - Premature birth: gestational age milestones, the developmental challenges associated with prematurity, and the typical clinical trajectory from admission to discharge - Kangaroo care (skin-to-skin for premature infants) and the research evidence on its effects on thermoregulation, brain development, weight gain, and parent-infant bonding - How parents navigate a NICU stay when one twin is admitted and the other is not — the emotional, logistical, and relational complexity of divided attention - The concept of anticipatory grief in NICU parenting: the persistent anxiety about outcomes and how it coexists with love and hope - How NICU discharge planning works and what the transition-home period looks like for premature multiples - How NICU experiences can affect parental mental health and what resources are typically available within and adjacent to NICU settings This course is organized as four readings. The first covers postpartum recovery specific to multiples. The second provides a comprehensive overview of the NICU environment. The third addresses the parent experience during a NICU stay in emotional and relational depth. The fourth covers discharge and the transition home. Reflection prompts follow each reading. This course is designed for expecting parents of multiples who want to understand the postpartum and NICU landscape before it arrives. No prior knowledge of neonatal care is required.
